She wakes up with asthma symptoms 1 to 2 nights a week; 2 puffs from her inhaler always brings complete relief of her wheezing and cough. Her peak expiratory flow PEF ; is 400 L min, which is 100% of predicted. Besides refilling her prescription for the albuterol inhaler, which of the following is the best management option for this patient? A ; Have her return for a visit in 3 months B ; Start therapy with a combination of a longacting - agonist and an inhaled corticosteroid C ; Start therapy with an ipratropium bromide inhaler D ; Start therapy with clarithromycin A 28-year-old woman comes to her physician's office with a 24 - hour history of continuous wheezing and dry cough. She is taking 2 puffs from an over-the-counter epinephrine inhaler every 15 minutes, with only partial relief. She used up her prescriptions for albuterol and fluticasone metered - dose inhalers, and she is requesting refills. Examination shows pulsus paradoxus decrease in systolic blood pressure of 20 mm She is using accessory muscles of respiration and is unable to lie flat because of dyspnea. PEF is 100 L min 30% of predicted ; before 3 aerosol treatments with albuterol and saline solution and 110 L min 37% of predicted ; after treatment. She reports feeling better after the treatments. Subsequent examination shows decreased breath sounds and no wheezes in the lung fields. Which of the following is the best management option for this patient? A ; Admit the patient to the hospital with a diagnosis of status asthmaticus B ; Refill her prescriptions and have her return to the office in 24 hours C ; Refill her prescriptions, prescribe prednisone 40 mg daily for 7 days, and re-evaluate in 24 hours D ; Refill her prescriptions, prescribe trimethoprim sulfate, and re-evaluate in 24 hours 3. She takes 2 puffs from an albuterol inhaler every 2 hours, with only partial relief. She ran out of her fluticasone and salmeterol 2 weeks ago. There is no accessory muscle use, and she has pulsus paradoxus decrease in systolic blood pressure of 5 mm She is afebrile, with a pulse of 90 bpm and respiratory rate of 20 breaths min. PEF is 100 L min 30% of predicted ; before and 250 L min 83% predicted ; after an aerosol treatment with albuterol and saline solution. The patient reports feeling better after treatment, but bilateral expiratory wheezes are heard in both lung fields. Which of the following is the best management option for this patient? Advice also needs to target the recipients' needs and respond to their concern or question, whether the recipient is a patient, a government minister, a committee, or one of millions of people listening to a broadcast or reading an article. How advice or opinion is couched and delivered will depend on the particular circumstances. | Formoterol vs salmeterolWe conducted a retrospective evaluation of the utilization and costs of asthma-related medications and health care services in patients with chronic asthma who were receiving inhaled corticosteroids and were newly started on salmeterol or a leukotriene modifier, using the claims-processing system of a large New England health insurer. A cohort of plan members with chronic asthma who were receiving inhaled corticosteroids was first identified through a review of all paid pharmacy, professional service, and hospital claims. Among these identified members, patients who were newly started on salmeterol or a leukotriene modifier i.e., montelukast, zafirlukast, or zileuton ; were selected for inclusion in the study sample. The date of the first paid pharmacy claim for one of the study medications was designated the "index date." The six-month period prior to the index date and the 12-month period subsequent to this date were designated the "pretreatment" and "follow-up" periods, respectively, for each patient in the sample. Utilization and costs of asthma-related care were then compared during the 12-month follow-up period between patients who received salmeterol versus a leukotriene modifier, including the use and associated cost of all asthma-related medications, outpatient services including physician office, emergency-room, and hospital outpatient visits ; , and inpatient care and glimepiride.

A possible limitation with any combination preparation is the lack of flexibility in dosing. This is not such an issue with combination inhalers as there are six different strengths of the fluticasone salmeterol products and two of the budesonide eformoterol products Table 1 ; . The eformoterol dose can be increased as the dose in these products is below the maximum that can be used, so patients can vary their dose during exacerbations. Another limitation is that patients may use the inhaler to relieve bronchospasm and get much higher doses of inhaled steroid than intended, particularly if they have an inhaler that contains a high dose of steroid. To avoid this, judicious prescribing is required with some thought given to the dose of steroid that and panadol and salmeterol.

Skin testing is used widely to determine the drug responsible for an anaphylactic reaction during anaesthesia. When a neuromuscular blocking drug is incriminated as the cause of a reaction, it is usual for neuromuscular blocking drugs which do not produce positive skin tests to be considered safe for subsequent use during anaesthesia. We describe three patients in whom false negative skin tests led to a second severe anaphylactic reaction to another neuromuscular blocking drug. We may be unable to protect our patents and proprietary rights. Our future success will depend to a significant extent on our ability to: obtain and keep patent protection for our products and technologies on an international basis; enforce our patents to prevent others from using our inventions; maintain and prevent others from using our trade secrets; and operate and commercialize products without infringing on the patents or proprietary rights of others. We cannot assure you that our patent rights will afford any competitive advantages, and these rights may be challenged or circumvented by third parties. Further, patents may not be issued on any of our pending patent applications in the U.S. or abroad. Because of the extensive time required for development, testing and regulatory review of a potential product, it is possible that before a potential product can be commercialized, any related patent may expire or remain in existence for only a short period following commercialization, reducing or eliminating any advantage of the patent. If we sue others for infringing our patents, a court may determine that such patents are invalid or unenforceable. Even if the validity of our patent rights is upheld by a court, a court may not prevent the alleged infringement of our patent rights on the grounds that such activity is not covered by our patent claims. In addition, third parties may sue us for infringing their patents. In the event of a successful claim of infringement against us, we may be required to: pay substantial damages; stop using our technologies and methods; stop certain research and development efforts; develop non-infringing products or methods; and obtain one or more licenses from third parties. If required, we cannot assure you that we will be able to obtain such licenses on acceptable terms, or at all. If we are sued for infringement, we could encounter substantial delays in development, manufacture and commercialization of our product candidates. Any litigation, whether to enforce our patent rights or to defend against allegations that we infringe third party rights, will be costly, time consuming, and may distract management from other important tasks. As is commonplace in the biotechnology and pharmaceutical industry, we employ individuals who were previously employed at other biotechnology or pharmaceutical companies, including our competitors or potential competitors. To the extent our employees are involved in research areas which are similar to those areas in which they were involved at their former employers, we may be subject to claims that such employees and or we have inadvertently or otherwise used or disclosed the alleged trade secrets or other proprietary information of the former employers. Litigation may be necessary to defend against such claims, which could result in substantial costs and be a distraction to management and which may have a material adverse effect on us, even if we are successful in defending such claims. We also rely in our business on trade secrets, know-how and other proprietary information. We seek to protect this information, in part, through the use of confidentiality agreements with employees, consultants, advisors and others. Nonetheless, we cannot assure you that those agreements will provide adequate protection for our trade secrets, know-how or other proprietary information and prevent their unauthorized use or disclosure. To the extent that consultants, key employees or other third parties apply technological information independently developed by them or by others to our proposed products.

As reported via an Anthem Prescription Product Update in April 2005, the FDA also issued a health advisory regarding a potential cancer risk from the use of Elidel cream and Protopic ointment. Used to treat atopic dermatitis a form of eczema ; , these two medications are the only approved drug products in the class of topical immunosuppressant calcineurin inhibitors. The advisory is based on animal studies, case reports in a small number of patients and knowledge of how drugs in this class work. Although FDA research continues, the long-term effect of Elidel and Protopic on the developing immune systems of infants and children is unknown at this time. Product labeling for these medications now reflects this warning. The FDA is currently working with manufacturers of these drugs to develop a Medication Guide MedGuide ; designed to educate patients and their families on the appropriate use of Elidel and Protopic. The MedGuides are expected to soon be available through your Anthem Rx Direct mail service or local retail pharmacy. During the week of May 9, 2005, Anthem Prescription will mail educational letters to more than 3, 700 customers who have used their prescription benefits within the past 120 days to purchase Elidel or Protopic for their child.
Using club drugs can cause a variety of health problems including changes in physical and emotional behavior. Drug abusers may exhibit exaggerated or unusual responses to their environment. The effects of drug use may result in central nervous depression, a state of excitability or a combination of both. Healthcare workers that are familiar with the signs and symptoms of drug use should utilize their ability to recognize drug related behavior and formulate a plan for the best initial approach to facilitate treatment. In an emergency setting when a drug abuser is unable, uncertain or unwilling to admit to the type of drug they have used, preservation of life and treating drug induced symptoms will always be the primary treatment goal. Stabilizing the patient by treating the symptoms and providing for their safety is the priority. Since toxicology results may not be available for hours or sometimes days ; , more specific treatment can't be ordered. The physician's treatment regime will be based upon the type of symptoms that the abuser is experiencing. Providing treatment for and communicating with individuals when they are under the influence of drugs should be approached with caution. Excited states of emotion can result in reactive behavior with highly exaggerated responses. Speaking in a calm and controlled tone of voice will help communicate to the user that you are in charge. The user might require frequent re-orientation to their environment and reassurance that they are safe. Nursing interventions that might help with providing care include: Setting limits and explaining all necessary treatments and procedures. Enlisting the support of a significant other or family member when appropriate ; . Decreasing outside sources of stimulation light, noise.
