17 HydroxyProgesterone 17OHP ; Testing The use of 17OHP has been recommended as a screening test for dogs with "atypical Cushing's syndrome." The definition of "atypical" is a dog with clinical signs and routine laboratory testing consistent with hyperadrenocorticism but with normal low dose dexamethasone screening test results, normal ACTH stimulation test results, and normal urine cortisol : creatinine ratio test results. Human beings, dogs and cats with adrenocortical tumors have been reported in which the primary hormone secreted by such tumors has been 17OHP. Adrenocortical tumors have long been known to synthesize and secrete a myriad of steroids and it is not surprising to learn that some primarily produce steroids other than cortisol. Such dogs and cats, in our experience, do not have "normal" screening tests results, but their results may be relatively low in cortisol. It is extremely rare for a dog or cat with PDH to produce only 17OHP. Further, the recommendation regarding use of this hormone involves assaying 17OHP after ACTH stimulation. Our recommendation would be repeating a low dose dexamethasone test if results are 0.9 g dl at the 8-hour sample, since the most common explanation for such a result would be administration of 0.1 instead of 0.01 mg kg of dexamethasone. If one is convinced that a dog has naturally occurring hyperadrenocorticism, and if that dog persistently has a non-diagnostic low dose dexamethasone test result, use of ACTH stimulation and assessment of 17OHP can be considered. This is an extremely unusual situation, however.

When type 2 diabetics were subgrouped according to the presence of autonomic neuropathy, higher gallbladder volumes were seen in patients with autonomic neuropathy. Conclusions : Cholecystomegaly, to a significant degree, was found in type 2 diabetics in the present study. It was significantly correlated with age, body mass index, and the severity of autonomic neuropathy. In male type 2 diabetics, gallbladder volume was significantly correlated with LDL cholesterol levels. In female type 2 diabetics, gallbladder volume was significantly correlated with waist-hip ratio. Isoflavone-Rich Soymilk Prevents Bone Loss in the Lumbar Spine of Postmenopausal Women: A Two-Year Study. Eva Lydeking-Olsen, * Jens-Erik Beck Jensen, Kenneth D. R. BACKGROUND: Previous short-term human studies showed a bone-sparing effect of soy protein with isoflavones at 80 90 mg d. No studies on the long-term effect of soy intake on bone mass have been published. AIM: The aim of the study was to evaluate the effect of 2 y consumption of isoflavone-rich soymilk 100 mg d ; , natural transdermal progesterone 25 mg d ; , or both on bone mineral density in the lumbar spine and hip. DESIGN: Postmenopausal white women, mean age of 58 y range: 4175 y ; , were randomly assigned, double-blind, to one of four treatment-groups: isoflavone-rich soymilk soy ; n 23 ; , transdermal progesterone TPD ; n 22 ; , both soy , TDP ; n 22 ; , or placebo isoflavone-poor soymilk, soy ; and progesterone-free cream TDP ; n 22 ; . baseline and 1 and 2 y, bone mineral content BMC ; and density BMD ; were measured in lumbar spine and hip by using dual-energy X-ray absorptiometry. All participants received a broad-spectrum food supplement Osforte ; containing 680 mg calcium citrate and carbonate ; , 300 mg magnesium aminochelate ; , 20 mg silicium sodium metasilicate ; , 15 mg zinc aminochelate ; , 6 mg manganese aminochelate ; , 3 mg boron proteinate ; , 2 mg copper aminochelate ; , 200 mg vitamin C, 40 mg pyridoxine, 200 IU vitamin D-3 and 1 mg vitamin K-1. The soymilk was enriched with calcium to 120 mg 100 mL; with a daily intake of 400 500 mL, total calcium intake from diet, soymilk and food supplement was 1200 1500 mg d. RESULTS: Of 107 women entering the study, 7 withdrew early, 6 developed intolerance to soy, and 2 developed intolerance to the skin cream. Three were excluded from analysis because of self-reported poor compliance, leaving 89 for analysis, 2223 per group. The percentage change in lumbar spine BMD and BMC, respectively, did not differ from zero in the soy group 1.1%, 2.0% ; and TDP group 1.1%, 0.4% ; but loss occurred in the soy group 4. 2%, P 0 01; 4.3%, P 0.01 ; and the soy , TDP group 2.8%, P 0.01; 2.4%, P 0.05 ; . No significant changes occurred in any group for femoral neck BMD or BMC, indicating that the food supplement, soy protein, or both had a stabilizing effect across the groups. CONCLUSION: Isoflavone-rich soymilk prevented bone loss in the lumbar spine and transdermal progesterone had a bone-sparing effect in postmenopausal women. The combined treatment, however, showed a negative interaction, resulting in bone loss to a greater extent than with either treatment alone but less pronounced than placebo.
