Background: The aim of this study was to investigate prevalence and risk factors for low back pain and pelvic pain LBPP ; during pregnancy. All women who gave birth at one of two hospitals of northern Sweden from 1 January 2002 until 30 April 2002 were invited to fill in a questionnaire on their obstetric history, recent pregnancy and delivery. Methods: The cohort was analysed by calculating the prevalence of LBPP during pregnancy. Univariate and multivariate logistic regression was performed to calculate odds ratio OR ; and its 95% confidence intervals CI ; where applicable. Non-parametric two-independent-samples testing was used to establish differences between groups. The response rate was 83.2% N 891 ; . Results: The prevalence of LBPP during pregnancy was 72%. Most cases reported both anterior and posterior pain. Increasing parity, history of hyper-mobility and reported periods of amenorrhoea were risk factors for LBPP. Women with LBPP had significantly higher pre-pregnancy weight, end-pregnancy weight, and pre-pregnancy and end-pregnancy BMI. Age at menarche and use of oral contraceptives were not associated with LBPP. Non-respondents were of the same age and parity as respondents. Conclusions: A majority of pregnant women report LBPP. Parity, low back pain and pelvic pain during a previous pregnancy, BMI, a history of hyper-mobility, and amenorrhoea are factors influencing the risk of developing LBPP during pregnancy, for instance, ondansetron tablets.

PSYCHOSIS IN PARKINSON'S DISEASE therapy are not considered consistent risk factors for hallucinations. Goetz et al.23 studied PD patients with a history of chronic hallucinations, noting that intravenous infusions of high-dose levodopa did not precipitate hallucinations. These observations suggest that the dopaminergic system is involved in the pathophysiologic process of hallucinations in PD, but that hallucinations themselves are not directly related to plasma and presumably brain ; levodopa levels. The mechanisms that underlie PD psychosis remain to be determined. The most frequently proposed hypothesis is that, in some patients, denervation hypersensitivity of mesolimbic and mesocortical dopamine receptors may occur and that dopaminergic medications may stimulate these receptors to cause psychosis.9, 24 More recently, it has been hypothesized that a combination or combinations of neurotransmitter systems play a role in the development of PD psychosis. Birkmayer and Riederer25 have suggested that a serotonergic dopaminergic imbalance may be most important. In support of this notion are postmortem studies revealing variable degrees and distributions of serotonin loss among patients with PD26 and reports that pharmacological agents that exert their effects on serotonin receptors e.g., atypical antipsychotics, ondansetron ; appear to have some efficacy against PD-related visual hallucinations.27, 28 Although deficiencies in cholinergic transmission are linked more closely to AD, there has been some suggestion that the ratio of serotonin to acetylcholine may be important for the development of psychosis in DLB.29, 30 Profound depletion of cholinergic markers and relative preservation of serotonergic markers in the temporal or parietal cortex have been suggested as a possible neurochemical explanation for psychosis in DLB.29 This would be in keeping with the finding that acetylcholinTABLE 1. Median number of emetic episodes between 5-122 . Cisplatin induces a biphasic pattern of emesis. The initial phase referred to as acute emesis is more severe and peaks over the first 6-8 h3 . The second phase or delayed emesis which begins 24 h after cisplatin infusion is pathophysiologically distinct from acute emesis 4 . The two most common antiemetic agents used with cytotoxic chemotherapy are metoclopramide and ondansetron5 . Ondansetron, a 5HT3 antagonist lacks the extrapyramidal side effects associated with metoclopramide, a D2 antagonist6 . Comparative studies of metoclopramide and ondansetron have been reported from the West7-12 but the same are few in our country. Ethnic, socio-cultural and educational level differences may play a major role in the attitudes towards acceptability of cancer chemotherapy as well as in the need for and response to antiemetic medication. Though pndansetron has proven to be superior to metoclopramide in the Western population10-11, the same may not hold true for the Indian population. In an Indian study by Advani et al13 , a wide dosage range of cisplatin 100-180 mg m2 ; and nonuniform chemotherapy regimens have been used making the extrapolation of results to cisplatin induced emesis difficult. Moreover many of these studies have used combination regimens, and the addition of dexamethasone has been reported to provide subjective improvement 14-15. There is a paucity of literature on the comparison of ondqnsetron and metoclopramide monotherapies in cisplatin induced emesis in Indian patients. There have been some reports on the dose related emetogenicity of cisplatin 16-20 but no studies have been done to compare the efficacy of antiemetics against the different grades of emetogenicity of cisplatin. In view of the paucity of information in Indian patients the present study was undertaken to compare the efficacy and tolerability of onndansetron with metoclopramide in acute emesis induced by 60mg m2 cisplatin either given as a single dose high dose regimen ; or in doses of 20mg m2 day over 3 days low dose regimen ; .
